The purpose of this survey is to collect data on the housing situation that forensic clients face and capture some of our efforts to address housing issues in our programs.

Use the following definitions when answering the next questions.
  • Affordable/Sustainable: Client can or will be able to afford the placement without the program long-term, either through their own income or through another funding source such as family care. OR in the case of program exit: The client is able to afford the placement either through their own income or through another funding source, such as Family Care.
  • Safe: Housing that does not jeopardize the health, safety, or welfare of its occupants.
  • Coordinated Entry Referral: Referral for a CR/OARS client to a housing program.
  • Homeless Coalition: Group of local agencies responsible for overseeing coordinated entry.
