Thank you for your interest in Department of Health Services' Tick Reporting and Identification Program. 


The DHS Vectorborne Disease (VBD) Program offers a service to identify ticks  you may find on yourself, your family members, your pets, or on wildlife (e.g., deer). Please use this survey if you are interested in learning about the tick you found or removed. Your responses to the survey will be kept confidential and all responses are voluntary.

Information Requested: This survey asks for your email address. The VBD Program will notify you of the species of the tick you submitted using the email address you provided with your submission. In the event the tick you found or removed is considered an invasive, newly emerging, or a tick species of interest, the VBD Program may contact you for additional information. Email addresses provided in the survey will not be used for any other purposes.
