Post School Outcome Survey - Students who are D/HH/DB (Exited June 2018)

As part of the OSEP (Office of Special Education Programs) process, districts are asked to provide information on students no longer in the special education program because they graduated, dropped out, or aged out. This survey is designed for former students who graduated, aged out or dropped out last year- June 2018, not for students currently enrolled in a secondary school program or transition program.

We are asking that you complete one survey for each of your former students. Unless otherwise directed, please read the question and ALL OF THE ANSWERS and mark ONE RESPONSE.
This survey is to be completed by May 4, 2019.

We want to be sure that the student is eligible for an interview. The next two questions will direct this. The survey will automatically move you to the correct place.
